We invest in communities to improve the Vital Conditions through projects prioritized by residents, giving voice and power to those who know their community best. The community-based work is carried out by local organizational partners that provide the infrastructure to facilitate community transformation.
Beyond investment, HCD influences a culture of equity through supports directly to partners and to the community at large.

We provide long-term, sustainable, and flexible funding to empower communities. By collaborating with diverse partners and focusing on underserved areas, HCD ensures resources are directly funneled to support resident-led change and community-driven revitalization.
We empower funded partners across Delaware who are working on aligned and related efforts. Our approach brings together resources and activities to build people-hours, skills, knowledge, and abilities with training and technical-assistance. Through liaison support, learning opportunities and coordinated supports, our HCD partners enhance their skills, knowledge, and abilities to better serve their communities.
HCD utilizes and employs Liaisons to strengthen community partners' efforts. Liaisons guide revitalization plans, connect partners to external resources, support the annual grant submission process, and provide technical assistance through one-on-one meetings. Each partner is paired with a designated Liaison to help advance their resident-driven initiatives.
HCD collaborates with national and local partners to deliver high-quality learning opportunities with organizations like NeighborWorks America and the Community Development Institute that uplift insights, tools, resources, and best practices for community-led and community-owned approaches. Partnerships provide robust skill development to empower community leaders.
HCD fosters new connections and strengthens existing relationships among experts, peer organizations, and resources. Collaborations with evaluation consultants and marketing professionals empower communities to showcase and share their impact through high-quality data and creative storytelling.

Institutes the Framework throughout Delaware in alignment with the Federal Plan for Equitable Long-Term Recovery and Resilience

Alignment of the Vital Conditions with the State Health Improvement Plan (SHIP) to address social determinants of health and support improved equity in partnership with State departments.
A two-date event that convened stakeholders, policy makers, and community leaders held to share knowledge of the Vital Conditions for well-being and explore “multisolving” to advance equity.
